To all heirs, beneficiaries, creditors, contingent creditors, and persons who may otherwise be interested in the will or estate, or both, of: Mary Ann Riddle.
A Petition For Probate has been filed by: Judy Stewart in the Superior Court of California, County of Lassen.
THE PETITION FOR PROBATE requests that: Judy Stewart be appointed as personal representative to administer the estate of the decedent.
THE PETITION requests authority to administer the estate under the Independent Administration of Estates Act. (This authority will allow the personal representative to take many actions without obtaining court approval. Before taking certain very important actions, however, the personal representative will be required to give notice to interested persons unless they have waived notice or consented to the proposed action.) The independent administration authority will be granted unless an interested person files an objection to the petition and shows good cause why the court should not grant the authority.

If you are a creditor or a contingent creditor of the decedent, you must file your claim with the court and mail a copy to the personal representative appointed by the court within the later of either (1) four months from the date of first issuance of letters to a general personal representative, as defined in section 58(b) of the California Probate Code, or (2) 60 days from the date of mailing or personal delivery to you of a notice under section 9052 of the California Probate Code.

This notice shall satisfy procedural requirements for activities to be undertaken by the Susanville Indian Rancheria Housing Authority (SIRHA).
REQUEST FOR RELEASE OF FUNDS
On or about January 1st, 2015, the SIRHA will submit a request to the HUD/SWONAP office for the release of Indian Community Development Block Grant (ICDBG) funds, to undertake a project known as the Yah-Monee Drive Project 2 at the cost of approximately $605,000 to be constructed on the Susanville Indian Rancheria, Susanville, CA.
FINDING OF NO SIGNIFICANT IMPACT
The SIRHA has determined that the project will have no significant impact on the human environment. Therefore, an Environmental Impact Statement (EIS) under the National Environmental Policy Act (NEPA) of 1969 is not required. Additional project information is contained in the Environmental Review Record (ERR) on file at the SIR Natural Resources Department (NRD), 735 Joaquin St., Susanville, CA 96130 and may be examined or copied weekdays between 8 a.m. and 5 p.m. during normal business hours.
PUBLIC COMMENTS
Any individual, group, or agency disagreeing with this determination or wishing to comment on the project may submit written comments to the Susanville Indian Rancheria Housing Authority Office, 870 Joaquin Street #G, Susanville, CA  96130. All comments received by December 31st, 2014, will be considered by SIR prior to the authorizing submission of a request for release of funds. Comments should specify which notice they are addressing.
RELEASE OF FUNDS
SIRHA certifies to HUD that Mr. Stacy Dixon, in his capacity of SIR Tribal Chairman, consents to accept the jurisdiction of the federal courts if an action is brought to enforce responsibilities in relation to the environmental review process and that these responsibilities have been satisfied. HUD approval of the certification satisfies its responsibilities under NEPA and related laws and authorities, and allows the SIRHA to use Program Funds.
OBJECTIONS TO RELEASE OF FUNDS
HUD will accept objections to its release of funds and the SIRHA certification for a period of fifteen days following the anticipated submission date or its actual receipt of the request (whichever is later) only if it is on one of the following basis: (a) the certification was not executed by the Certifying Officer of SIRHA; (b) SIRHA has omitted a step or failed to make a decision of finding required by HUD regulations at 24 CFR Part 58; (c) the grant recipient has incurred costs not authorized by 24  CFR 58 before approval of a release of funds by HUD; or (d)  another Federal agency acting pursuant to 40 CFR Part 1504 has submitted a written finding that the project is unsatisfactory from the standpoint of environmental  quality. Objections must be prepared and submitted in accordance with the required procedures (24 CFR Part 58) and shall be addressed to the HUD grant administration office at Department of Housing and Urban Development, Southwest Office of Native American Programs, One North Central Avenue, Suite 600, Phoenix, Arizona 85004. District Background
Lassen Union High School District is home to two (2) school sites and a district office. Our students range from 9th through 12th grade.
School Districts in Lassen County that elect to enter into a contract to provide energy planning, auditing and implementation services will enter into a separate specific contract approved by their respective Boards of Education. These School Districts may, or may not, enter into a contract with the successful energy services company. The following school districts may elect to participate in this process (piggyback): Janesville Union School District, Johnstonville Elementary School District, Susanville School District, Richmond School District, Fort Sage School District, Long Valley Charter, Westwood Unified, Westwood Charter, Big Valley Unified, Shaffer School District.
Scope of Work
NOTICE IS HEREBY GIVEN that pursuant to California Government Code 4217, Lassen Union High School District intends to use a competitive process to select a qualified firm, based upon best value, to provide design-build comprehensive energy management and energy-related capital improvement services. The improvements will be aimed at reducing energy consumption associated with the heating, ventilation and air conditioning system, the lighting systems, building envelope, and other energy consuming devices and systems, as well as through possible deployment of renewable energy systems. The selected firm will assist the District in developing and implementing an energy master plan that incorporates appropriate utilization of the District's Proposition 39 allocation(s) as well as other relevant funding sources. The selected firm will provide a turnkey solution with services including, but not limited to:
Project consulting and planning
Project administration
Investment grade technical energy audit
Financial analysis, rebates and incentive support
Engineering design
Acquisition
Installation
Modification
Commissioning
Maintenance Support
Monitoring
Training in the operation of existing and new equipment

The selected firm shall be the single point of contact on Prop 39 related projects and shall be responsible for assisting Lassen Union High School District with the development and approval of a Prop 39 Expenditure Plan. Upon approval of the Expenditure Plan, the selected firm shall serve as the prime design-build contractor and single POC for all implementation work associated with the approved Expenditure Plan.
RFQ - Proposal Format and Submission
The proposal shall address the following items in the order listed below and shall be numbered in the qualifications submittal to match the following list. The firm's qualifications shall be limited in length to not more than 25 double-sided pieces of paper (excluding cover letter, appendices and tab separators).
I. Cover Letter: The letter should include a brief summary of the firm's experience, the unique qualities of the firm and its personnel, what will make the firm a good fit for the District for the proposed contract and must bear the signature of the person(s) authorized to submit the proposal on behalf of the firm.
II. Qualifications: Please describe the firm's areas of expertise and how the firm's expertise will benefit Lassen Union High School District.
a. Identify the Proposed team who will be working with Lassen Union High School District on the project. Include an Organizational Chart and indicate the roles and responsibilities of each individual, including each individual's qualifications and experience with similar projects.
b. Provide 3 Educational or Institutional references of Projects with similar size and scope to the proposed project.
c. Please include the name, address, and telephone number of persons who can attest to the firm's performance on the described projects.
d. Describe your firm's experience with Prop 39 projects. Preference will be given to firms who have or are currently working with other universities, community colleges or K-12 customers to develop and implement Prop 39 projects.
e. Describe your firm's financial capacity to handle the project. Include company bond rating, bond limits, and evidence of insurability.
f. Provide a Letter from Bonding agent stating Firm's Bonding Capacity.
g. Provide a Certificate of Insurance.
h. Provide California General Contractor's license number.
i. Describe your firm's approach to safety.
j. Describe any unique capabilities that your firm offers.
III. Project Approach
a. Describe your firm's approach to securing and maximizing Prop 39 funding
b. Explain your firm's approach to identifying energy efficiency measures and the process associated with recommending energy efficiency measures.
c. Describe in detail your firm's methodology for calculating energy cost savings.
d. Communicate Firm's Construction Implementation plan for a School
e. Provide a description of your firm's commitment to obtaining OPSC Project Closure status, as well as experience with DSA, for public school projects.
f. Describe your firm's approach to Measurement and Verification of Energy Savings and any guarantees associated with Energy Savings.
g. List experience working with third party funding, including but not limited to state, federal, and utility funds, grants, rebates, incentives, etc.
h. Discuss any Financing options, and procurement options that your firm provides
IV. Cost Structure: The District intends to award individual Task Orders under a Master Services Agreement based on timing of Prop 39 and other funding sources and project development cycles.
Please describe your pricing process and engagement model. For evaluation purposes, please provide the actual percentages, labor rates and costs that your firm proposes to enter into with the Lassen Union High School District:
a. Percentage of profit
b. Percentage of indirect expense (overhead)
c. Hourly rate table (including fringe benefits) of professional services personnel
d. Hourly rate table (including fringe benefits) of construction management personnel

Proposals will be evaluated by a Lassen Union High School District committee comprising various representatives of the Lassen Union High School District. This committee hereinafter will be referred to as the "Review Panel."
The Review Panel will review each Proposal and determine the following:
1. Whether each submitted Proposal is responsive to the requirements of this RFP.
2. Determine the "Best Value" submittal. The Proposer with the highest "Best Value" score will be selected by the District, as the best responsible proposer.

Notice of Availability for Public Comment on the Draft Environmental Impact Report (EIR) for the Water Quality Certification (certification) of Pacific Gas and Electric Company's (PG&E) Upper North Fork Feather River Hydroelectric Project (UNFFR Project), Federal Energy Regulatory Commission (FERC) Project No. 2105.
The UNFFR Project is located in the upper North Fork Feather River watershed upstream of Lake Oroville in Plumas County, California. The UNFFR Project was originally licensed by FERC in 1955 and is currently undergoing relicensing. The UNFFR Project includes three reservoirs with dams (Lake Almanor, Butt Valley reservoir, and Belden forebay); five powerhouses; tunnels and penstocks connecting the reservoirs to the powerhouses; transmission facilities; access facilities; and associated operation/maintenance activities. The UNFFR Project has a total nameplate capacity of 362.3 megawatts.
The proposed UNFFR Project consists of operation of the UNFFR Project under a new FERC license in accordance with PG&E's application to FERC, a settlement agreement, conditions proposed by the National Marine Fisheries Service and the United States Forest Service, and FERC's Staff Alternative. The Draft EIR also includes an evaluation of two alternatives. Generally, both alternatives involve construction of thermal curtains at the Prattville and Caribou intakes along with the activities described for the proposed UNFFR Project. One alternative also includes increased releases from the low-level outlet at Canyon dam from June 15 to September 15, which would require modifications to the Canyon dam outlet structure. 
The Draft EIR identifies significant impacts for the following resources: Aesthetics; Air Quality; Cultural Resources; Fisheries; Geology, Geomorphology, and Soils; Hazards and Hazardous Materials; Land Use and Mineral Resources; Noise; Recreation; Transportation and Traffic; Vegetation, Wildlife, and Sensitive Biological Resources; and Water Quality. All significant impacts can be reduced to a less than significant level with implementation of mitigation measures, except for Aesthetics, which is identified as significant and unavoidable.

2014 ROOSEVELT POOL DEMOLITION PROJECT
Will be received at the HLVRA c/o City of Susanville, Attn: Jared G. Hancock, 66 North Lassen Street, Susanville, California 96130, prior to 2:00 PM on December 18, 2014, at which time they will be publicly opened and read in the City of Susanville Council Chambers at the above address.
General Work Description: 
CONTRACTOR shall, at its sole cost and expense, furnish all facilities, equipment, and other materials necessary for the demolition and removal of the Roosevelt Pool and Structure located at 800 South Street in Susanville, California. This building was constructed in 1937 and consists of approximately 11,730 square feet of timber/masonry type construction. Asbestos and lead based paint removal will be required as described in the attached document "Asbestos and Lead-Based Paint Evaluation Structural Demolition Project.
Work will also include the removal of certain deciduous trees adjacent to the structure as directed by the Project Engineer; abandonment and removal of an exterior utility pole located near the southeast corner of the structure and protection of an interior existing geothermal well located near the southeast corner of the structure located in the basement area.
Other subterranean utilities near the southeast corner running easterly and an underground fuel oil storage tank (location to be determined) are to be removed and abandoned.
Contractor shall coordinate with all affected owners of utilities and adjoining properties inclusive of discovery and/or investigation of said facilities, proper preservation (if so desired), procurement of all required permits and inspections and/or testing to facilitate the desired removal or abandonment.
At the time this contract is awarded, the Contractor shall possess a California Class A, B or Class C21 - Building Moving/Demolition license and in addition the Contractor or the Sub-Contractor who is responsible for the removal of hazardous material shall possess an ASB - Asbestos Certification and a HAZ - Hazardous Substance Removal Certification.
This contract is subject to the provisions of Section 22300 of the California Public Contract Code, which provides for the substitution of securities for any monies withheld by a public agency to ensure performance under a contract. The bidder's attention is directed to said Section 22300 of the Public Contract Code located in the specifications for the specific requirements and provisions for such substitutions of securities if requested by the Contractor.
This contract is subject to nondiscrimination and compliance requirements pursuant to Government Code, Section 12990.

Changes discussed during the pre-bid meeting will not be effective unless it is made in writing as an Addendum, at which time it will be sent to all plan holders.
